Rare case of pediatric tuberculous meningitis in a 2-year-old: A diagnostic challenge

Abstract
Tuberculosis (TB) remains a leading cause of morbidity and mortality globally, with children comprising 12% of all TB cases in 2023, according to the World Health Organization's Global Tuberculosis Report. Despite this, pediatric TB has been underemphasized in global TB control strategies. TB in children is particularly challenging due to high transmission rates, insufficient diagnostic tools, and the overlap with other common childhood illnesses. Tuberculous meningitis (TBM) is a severe complication of TB, more common in children due to their immature immune systems. TBM is associated with high mortality and long-term neurological sequelae, often presenting with nonspecific symptoms that delay diagnosis. This case report highlights a rare instance of TBM in a 2-year-old child in a low-incidence country, emphasizing the need for increased clinical suspicion and improved diagnostic methods. The child’s initial presentation was nonspecific, leading to a delay in recognizing tuberculous meningitis and progression of neurological symptoms. TBM was confirmed after neuroimaging and a positive QuantiFERON Gold test. The patient showed gradual recovery with anti-tuberculous treatment. This case underscores the importance of TB screening, particularly in children with a known exposure to TB, and the necessity for prompt diagnosis and treatment to prevent severe complications.
